I would have the wound cultured and probably have mother use antibiotic ointment (very very clean hands or clean cotton-tipped applicator to apply) about 4x/day after washing with mild soap and water and rinsing and patting dry at least 2x/day. and I would recommend oral antibiotics based on wound culture.
