Respectfully Liz, I don't think it is as clear cut as all that. Of course many women who end up having IGT would like to have known before their baby was born that they would not be able to make enough milk...but there is no way of knowing beforehand. And implying that there is something wrong with a woman's body before it is evident is not a benign intervention. I get that where the health care system is awful and women don't get good follow up after birth that anticipatory guidance is particularly important but I do not agree that women who have "funky looking" breasts should receive different anticipatory guidance in terms of what's normal and what to do if things are not normal than mothers with typical looking breasts.Karleen GribbleAustralia

What is wrong with suggesting a hypoplasia-looking woman spends the first 3 days doing some extra hand expression or pumping and keeping a close eye on the baby's pees, poops, and weight? If she doesn't have IGT, then she just spent some time collecting an emergency supply of milk. As Benjamin Franklin said, "An ounce of prevention is worth a pound of cure."
