I work in a small hospital, so we may be a bit more informal than others.  If
I am speaking with the physician in front of a patient, I use "Dr. *****".
If I am speaking with the physician face to face or on the phone and s/he is
someone I regularly work with, I use first name.  If I do not know the
physician well or at all, I resort to the "Dr. *****".  In any
mass correspondence that goes out I use "Doctor *****" in the salutation and
then draw a line through it and hand write the first name for those I know
leaving it as is for the others.  If it is a thank you note that is more
personal and hand written, I use either first name or formal per above
criteria.  Anytime a physician refers to me as "Pardee" I refer to her/him as
first name.  No one has complained.  I am not a nurse, so I do not know any
better.
