I wonder, Jennifer, if the feed you observed was better than normal.  It sounds like baby's feeds have been marginal (some good, probably worsening with fatigue throughout the day) and provided enough milk to keep baby gaining slowly.  That coupled with mom's milk supply compensating for baby's poor feeding ability.  Latch can look okay and not be adequate.  That could explain the larger bottles during separation...baby is feeding better with the bottle and gets his calories then, then goes back to marginal feeding on mom.  Just at thought.  A suck assessment with and without flow would be very beneficial.  Good luck!
