Hello everyone!
Our hospital-based Lactation dept. offers a "Breastfeeding Warm Line" to any mother with breastfeeding questions. Many of the mothers who call do not identify themselves (nor do we ask for that info) and many are not even patients at our hospital.
We document every phone call in a log book (re: information we are given by mother and our suggestions) and maintain the records indefinitely. If an issue is concerning, we document in the pt's chart in the form of an e-message to the pt's MD. If the mother is not one of our patients, we give the mother verbal recommendations to seek medical help.
I am wondering if any of you would share how your "warm lines" operate in terms of documentation?
