Just one more idea to throw into the mix.... with all the problems 
you describe, and this being mom's fourth baby, thus obviously a very 
busy lady... I'm wondering if the baby's weight gain is good so 
far?  ie a month-old baby should have gained ~800g over the 
birthweight?   I've found, when working with low-gain/FTT babies that 
the challenges listed such as shallow latch, infection, mastitis and 
now severe nipple pain could also be pre-disposing factors for 
inadequate milk transfer, and that when this is the case the babies 
are often extremely tense, with very high muscle tone.   So just 
wondered if this could also be a reason for the clamping you describe?

all? (She does not suffer from Raynaud's Phenomenum)Would there be 
any issue in taking nifedipine while she is taking the Fragmin?
