When I started up a group, I made the mistake of doing it in a clinical setting to start with. We have since moved to a church location with a playground which makes it more flexible for those with older children.
The other aspect that I think is important is to have a virtual group on facebook or some other such entity to help the moms connect outside of in-person meetings. It is possible to set up a "secret group" on facebook so that all the friends/family of mom don't get more info than she wants to share about her breastfeeding experience :-) It also breaks down barriers of coming to a group of "strangers" if a mom has met them online first.
