I think the basic problem world round is "not enough milk."  Mothers simply don't get it that a newborn nurses  frequently.  
Baby knows I need to nurse  every time I think of it to make my mom's supply robust, but my mom thinks it is too often, therefore mom thinks there isn't enough milk. 
Part of that problem comes from the bottle feeding culture that surrounds us on all sides. 
 
Another common problem is if mom has any difficulty with anything,  breastfeeding is blamed.  Pat in SNJ
