If I am reading you right, this 3 wk old baby is getting approx 5 ml per feeding plus a 2 oz bottle of formula DAILY. This is starvation and baby needs to be fed generously immediately. Unless I am reading you wrong.??? Sorting out the myriad of reasons for low supply are secondary.
Unfortunately we see many moms with low milk production. One thing I think is very important for LCs and parents to realize is:
a baby will pee and poop 1 or 2 x per day in the first 2 days, even if not eating a thing! They are born with excess fluid and a belly full of meconium. Often when I've identified a baby in hospital with poor milk transfer or not enough feeding, the parent will say "but he is pooping and peeing like the chart." That is fine, but the baby should INCREASE the number and heaviness of the pees and poops once at home, not decrease to no stooling.
