Hi, Mary,

As far as I know, there is nothing smaller available than 21mm.  Pumpin'
Pals doesn't make anything that small, but the opening is tapered rather
than sharply angled, so it's possible it might be more comfortable.  She
could also try using an extra small nipple shield over her own when
pa\umping so the pump can only stretch her nipple as far as the shield and
it won't be yanked into the flange.
