Rachael you are right (mores the pity!), nor are reviewers paid (again more the pity- I've reviewed for Pediatrics several times and the cheque is not in the mail), and often not editors either (knowing well a few hard working ones). The same usually goes for book chapters- having just spent a month writing one, I can expect a free copy of the book in payment.
