Hi Francoise,

When I hear of a mother with apparent overactive milk ejection but not
overproduction of milk, I suspect that actually the baby (or twins in this
case?) is having an abnormally difficult time managing a normal flow.

I would look for things like a subtle tongue tie, or airway problems in the
baby which are making the baby struggle when the milk ejects. As you may
already know, a baby can have a tongue tie and still appear (on the
outside) to have an excellent, wide latch.

When you say the suck is good (although rapid), have you felt it with a
finger in baby's mouth, or only seen how it looks on the breast?

Until it is fully understood what is causing the babies to show difficulty
during milk ejection, I would stay away from any practice that would reduce
the mother's milk supply (such as block feeding). A baby that cannot manage
normal flow may not have an easy time appropriately stimulating milk
supply, and may really need the mother to have a very abundant supply in
order to grow well.
