We advise our preterm moms to first use their colostrum and then fresh milk whenever possible before using frozen milk. The HMBANA guidelines say use fresh milk whenever possible and then colostrum and then the oldest milk. It makes sense to me that a mom be using fresh colostrum/milk whenever possible because that's what the baby would get if s/he was directly breastfeeding.
What do you do at your facility and what is the rationale?
