Just focusing on your question rather than asking what are the feeding issues, can thickening be avoided, and looking at pros/cons of thickening...  :)
Human milk usually doesn't retain thickening, so it needs to be thickened *at* feeding time.  And how about using thickening agents like xanthum gum, simplythick, etc. instead of rice?  I'm actually a little surprised at the suggestion of rice at all.
