Psychological states such as "intention," "motivation," "commitment," and "determination" play a huge role in the initiation of breastfeeding and in the feeding-by-feeding  choice to continue or not continue breastfeeding. These psychological states are undermined as women encounter significant barriers to the development of the breastfeeding relationship. As important as it is, a ll the understanding in the world of the physiology of the mother-baby dyad and  of the techniques and tools to move  the physical act of breastfeeding closer to and into the spectrum of biologically normal will be of little to no use if IBCLCs do not have the time, for whatever reason, to appropriately engage with and effectively respond to the psychological experience of the mother and baby . The b reastfeeding relationship is the biologically normal means through which  the physical and psychological intertwinement of a mother and baby is further developed and expressed after birth. When that relationship has been significantly  injured , the provision of compassionate and effective  services for its healing takes great skill and much time.
