Fat necrosis is a rare consequence of severe breast injury. Lactation requires cell signaling interactions between fat and glandular tissue, especially during pregnancy. Perhaps there is vascular damage also. Does mom have access to a good breast surgeon?

PTP- a first time mom who was hit right in the middle of her breast by a
baseball when pregnant about 7 months. She had swelling and bruising at the
time. Now her baby is 9 weeks old, and you can still see traces of the bruise
although there is no swelling. Problem is, she has had pain in that nipple from
the start- mostly at the start of the nursing. The pain does not come at other
times, except when she pumps. She has tried different sized flanges.
The nipple is not deformed after nursing. The baby
does not have tortocollis or any obvious assymetries that I can see, and is
gaining well.
The breast with the painful nipple produces less milk and has had mastitis once,
with some bleeding. I know, sounds like the latch is not so great on that side-
but the mastitis could also be from her avoiding nursing d/t pain.
Here's my question: Is it possible that the injury is causing the pain? Has
anyone seen something like this?
