Pat Bucknell asks for input for a colleague not on LN.
This does not sound like dilation to me, sounds like Raynaud's, which is vasospasm with dilation and constriction.  Seen often after prolonged soreness/painful feeding. Would be very surprised if cold was helpful, would sooner try heat myself.
The vasoconstriction could impede flow - as could pain bad enough to inhibit MER.  I'd look in baby's mouth and carefully watch jaw/cheek/lip action during a feed to try to identify any factors that are causing the pain and the vascular reaction.
