Sometimes it's not lipase -- it's a change in mother's diet (including a change in water).
I'd share this easy rundown from kellymom & see if anything rings a bell with the mom. For example, the hard water and fish oil supplements. Even though mom says both are consistent there can still be variables (perhaps dosing or sources fluctuate). Also, nothing about human milk remains constant -- and it's not exactly homogenized! ;)

The mother may want to reconsider her choices for water, water filter, supplements just to see if that makes a difference.
