And hyperglycemia only slightly raises the level of glucose in the mother's milk, because very little glucose is transferred into milk. The level of lactose is controlled by alpha lactalbumin, not by glucose availability, so that is unaffected as well. 

Mild ketosis should not matter, ketones are an alternate fuel and not toxic until levels are high. Newborns produce ketones as part of their metabolic adaptation to episodic feeding after birth (and lactate too) to keep their brains going while moms milk production increases. As Nikki said, severe ketoacidosis would make mom incapable of child care until she's treated. It's the acidosis which is most dangerous for mom.

As for ethanol, there is controversy on how many drinks a mom can have without waiting to breastfeed again. One drink does not necessarily require not bf for a few hours.
