Hi Trisha,
Yes, quite the challenging case. The priority would be to increase the
intake which your team is working on as best as possible, and then I would
be thinking of these things:
--allergy? desperate times call for desperate measures and I would be
willing for a trial of a truly elemental formula, completely hydrolyzed (I
think Elecare or Neocate?) to see if the skin problems resolve quickly and
baby gains. Then you would know it is a food allergen and begin the tedious
process of finding that out.
--skin allergy / contact ? I would encourage kangaroo care for weight gain
but first I would try to eliminate all scents, fragrances, etc in contact
of mom and baby, maybe using some very mild soaps, washes for baths and
laundry, double rinses etc.
-- some type of metabolic derangement or primary vitamin deficiency in
which some part of the pathway(s) is abnormal. If there is a deficiency due
to poor intake then the increased volume/calories should correct the skin
issues and the metabolic could be ruled out.
