The shield helps shape the tissue into a more manageable size that the baby can accommodate in its mouth.
It really does work in some cases. I imagine if you have never seen it work how it might be a mystery how it would be helpful but it can be a useful intervention in some cases.
The shield is cone shaped, and often the mother's large nipple is not. Applying the shield shapes the tissue into the cone shaped stem of the shield and the baby can actually get their mouth around it.
It's not a solution for every case, but it can work for some cases.
