Hi, Rachel,
If mom has enough milk, you can have her set aside the first letdown, then
save the remainder of the pumping session for feeding to increase the fat
content and make it thicker. Have her do a pumping diary. You might find
that she's sleeping a long stretch at night and then bringing in the first
pumped milk because there is a larger volume than her other pumping
sessions. Also, if they are putting tube feeds on a pump, make sure the tip
of the syringe is pointing upward so the fat is not sticking to the sides.
The fat rises and will be delivered first.
I would also check for tongue tie which is frequently associated with
reflux. Delivering smaller feeds more frequently may also help.
