Just recently at the hospital i work at we have stopped using a consent form to supplement. It was in use for a long time but it managed to become nothing special. The form which was suppose to be filled out at the time a baby needed supplement or mother wanted to supplement started to end up being added into the all the paperwork that was signed as soon as the parents arrived at the hospital so every baby ended up with a signed consent form. When I brought up the fact that the paper was being used in this fashion it was determined by the pediatricains not to use this form at all because it 'stresses parents out' and makes them feel 'guility'.
Like the saying goes ' One step forward two steps back'.
