I haven't peeked yet at the ICD10 codes, but currently I am billing an E/M visit and get much better reimbursement on baby, than mom. If I document on both those, I bill for both.
Diagnostic codes I've used are the standard V20.1, V20.2 and 783.3 (feeding difficulty), 796.1 (suck reflex abnormal), 779.31 (feeding problems), V67.59 (follow-up to resolved problems), 520.7 (abnormal weight loss), 750.1 (abnormal tongue position), and 771.7 (neonatal candida infection).
For mom, the standard V24.1 or anyone of these:
676.24 engorgement of breast (not billable)
676.04 inverted nipple
676.14 cracked nipple
675.24 mastitis, nonpurulent
675.14 mastitis, abscess
676.34 nipple trauma
I don't see the frenotomy/frenectomy codes on my cheat sheet, although I have used them with success in the past. I'll have to dig those up and send at a later point.
Blessings,
Penny Lane MSN, CNM, IBCLC
