I apologize if this was already mentioned but has anyone checked this baby for a cleft in the soft palate?  Your case sounds eerily like a baby I saw a few years ago.  This baby was full-term, vigorous but could not draw from breast, bottle, finger-feeder...I thought I felt a cleft during an oral exam, mentioned it the ped, got pooh-poohed, baby ended up in the NICU gavage fed and, voila, the next day a NICU nurse noticed the cleft when baby was crying.  This babe just could not make suction.
