This question continues to trouble me, too.  Does anyone know if telling a mother to add powdered (or ready to feed) to her breastmilk is considered "off-lable?" or something?  I see mothers routinely told to add the 22KCal stuff to their milk for three or more months based on pramaturity at birth or just late preterm....  Great way to undermine breastfeeding.
