I work with a WIC peer program and have also have mothers who've told peers the same thing - that the baby takes formula but not breastmilk from the bottle. We've had a couple of theories. One is that baby associates the breastmilk with mom and so will refuse the bottle because he/she wants the real thing.
Since this mom is exclusively bottle-feeding though I'd second Judy's suggestion that it may be flow-rate. Formula has a thicker consistency and so flows slower from the bottle so may be more comfortable/controllable for the baby.
