I came into this story late, and don't recall and can't find your original
post, so this may be out of line, but do you mean she never had any fullness
in the first week postpartum ever?  Did she have any breast changes in
pregnancy? I wonder if she has rare but true lactational insufficiency, and
all the beer and brewer's yeast and reglan and pumping and hungry baby is not
going to do it for her, because she just doesn't have the breast tissue for
it.  What do her breasts look like? Hormonal, pituitary issues are also a
possibility if she really never experienced her milk coming in but I would
ask some of the more experienced folks online to comment on that before I
started barking up the wrong tree.
