Hi Zena,
I have seen and felt very firm and round breasts that I swore had implants but they did not. I would not expect that the implants had migrated from behind the muscle.
The mother may in fact have implants in front of the muscle or her breasts just feel that way during lactogenesis II. With either type of implant placement mother should be counseled to practice unrestricted bf, and anticipatory guidance provided to her re engorgement prevention/recognition/treatment if needed. In my experience, I do find that mothers with implants have more trouble with engorgement.
