This might be different for an MD, but when I was a hospital-based LC, we were
told to code all visits as "infant feeding problem". This was
because supposedly that made it more likely that insurance would cover the
visit. (Apparantly insurance didn't think it was important for mom to be able
to nurse, but, of course, it was important that babies feed well. But that's
another issue.) We justified that approach because even if breast/nipple pain
was the initial complaint, most often it was secomdary to ineffective feeding.
Even if the infant appeared to be nursing adequately and getting enough milk, if
the problem was not resolved, it was likely to lead to less effective or no
breastfeeding.
