At my hospital, when talking to the nurse manager about getting frenotomies done in our nursery, I framed it as a patient satisfaction issue, that parents who want the procedure done, should be able to get it done.
I have found a moderate way to discuss it with parents, because after all, it is the doctor's call. I show parents the tight frenulum discuss lower, deeper, latch, and very close positioning, mention that it can be clipped and is helpful in many cases, and to discuss with their dr and do some reading about it. Then I document in baby's chart what is happening, if feedings are painful for mother, that the full extension of the tongue is limited.
