Rachel, you've pretty much pegged it.
"Consent forms" are meant to document that a matter has been discussed, and
the patient is fully informed about the risks/benefits of an action to be
taken.
"Waivers " (often less-accurately-referred-to as "consent forms") are meant
to document that a matter has been discussed, and the patient **waives** the
right to future complaints or crankiness about an action to be taken.
