One possible source is a nipple that folds over inside the bra. If a crack
at the base gets started, every time she "puts her breast away" the nipple
may fold at its base and stay wet. I don't quite see how it would fissure
*all* the way around from that, but maybe if it folds differently at
different times? Just wearing bra flaps down would take care of that.
And there's what Chele Marmet calls an "intussuscepted nipple". It's as if
the nipple shaft needs to heal in the "open" position, but after every
feeding it shortens on itself, closing the fissure and preventing its full
surface from healing over. At every feeding it rips open again, only to
close afterward.
I've seen a few of these. When the skin finally heals - usually with shells
or a dimple ring or something that will keep the wounds "open" as they heal
- the scar tissue may be visible thereafter, and the shape of the nipple may
be slightly different from what it was before, but it doesn't cause further
problems.
