Brenda, are you in the US? In the US, physicians risk loss of licensure for prescribing domperidone because it is not approved for any use. To the extent it is within the scope of practice of IBCLCs to discuss prescription medications with clients, I see no legal harm in telling a mother the drug exists. However, it can not be lawfully obtained in the US (and before the hate mail starts, I am well aware that it is indeed readily available through compounding pharmacies and the Internet - availability and legality are very different). Directing a mother to obtain a drug unlawfully carries with it legal risk and is ethically questionable.

The change to orphan drug status (anyone have a copy of the paperwork on that? I can find nothing on the FDA site about it) is the beginning of a long process that may or may not result in approval of domperidone for sale in the US.
