Hi Moe,
A few thoughts on this case. With the history you describe, I would
want a very accurate naked weight. I would want frequent follow-up
including accurate weight, for example, every day or two. I would want
baby to continue breastfeeding with what I call switch nursing
(keeping baby feeding actively and switching sides if she slows down
too much) with breast compression.
I would want baby to be supplemented GENEROUSLY with mom's previously
expressed milk. I like lactation aid at breast when it works. I would
ask mom to express regularly as much as possible until situation
resolves. I would discuss with mother what to expect as far as
effective feeding, good urine and stool output, and usual weight
patterns. She needs the frequent followup as much as baby does. Some
moms get so defeated and guilty when they find out baby is not getting
much milk in, and some moms are in denial that anything is wrong, some
are oblivious and feel that sleepy, "agreeable" babies are great, or
anything in between. So it's a journey for her too and she needs
support.
