 If I were seeing this baby I would be having a serious discussion with all the health care providers involved and if that mother were not taking measures to ensure the baby was properly fed, I would have a duty to report to the Children's Aid.   I have only ever done that once and it was a great relief to me when I knew that baby was being properly cared for.       Rachel is right---This is a disaster.    There is a possibility that baby has some underlying disease process going on which prevents weight gain.   Perhaps the mother has a severe postpartum depression which manifests itself in an inability to see her baby is starving.    Whatever is going on , this is totally beyond the professional capabilities of an IBCLC.      Please get help for this baby before there is irreparable damage.      Eileen
