Yes! Thank you. As a result of the mPINC scores, the DHS is actively helping a great number of hospitals in my state to improve BrFdg rates and outcomes, they are teaching 5 of the 10 BFHI Steps. Obviously, one is the use of pacifiers. With this article they are saying now, "Why is it that we need to discourage pacifiers? This Cochrane Review says pacifiers do not affect BrFdg rates or duration."
Many staff feel the only real effect of avoiding pacifiers is to make life difficult for RNs and new moms who are tired of BrFdg "all the time". Well, you already know that drill. I'm preaching to the choir on this list.

My thoughts on pacifiers is: if the study does not control for the TYPE of pacifier used (shape and length) with consistency between the groups, it is not a valid study. In my experience, many of the babies with breastfeeding problems are using flat or weirdly shaped pacifiers that encourage biting and not sucking. It's like comparing bottles and breastfeeding - it all depends on which mom, which baby, which bottle, etc.
