Go to the California Perinatal Quality Care Collaborative, or 
<www.cpqcc.org>   and search for their Nutritional Support for VLBW babies Toolkit. 
Dr. Wight was one of the authors of this protocol so you know it is very well documented.
It was updated this year 2010.
