There are far safer ways to treat this than synthetic hormones. I would suggest she see an ND--there are plenty in NYC. I have known moms threatened with surgery who treated with calendula. If I had been asked this by a friend, for example--I would tell her to soak every couple of hours with calendula tincture diluted 1:10 and then apply tamanu oil afterwards, as well as every time she pees. Calendula promotes healthy granulation and tamanu oil breaks down keloids and prevents scar tissue.

Subject: Premarin Cream and Breastmilk
LactMed is being unusually not helpful, so I turn to you. (Permission to post)
A client who has been exclusively pumping for 4 months and has a great supply
and in fact pumps so much milk she has bought an extra freezer just for
breastmilk has had some troubles healing from her delivery. She has developed
granular tissue in the vagina and her GYN has prescribed vaginal premarin cream
for application 2-3 times a day for 2 weeks prior to surgery. (I'm finding out
the dosage now).
She is very reluctant to start the cream because she doesn't want to negatively
impact her milk supply and knows that estrogen can. Nor does she want to
negatively impact her daughter's development by introducing estrogen. Baby is
about 4.5 months and 18lbs. Has anyone had experience with a mother in a
similar situation?
Thank you in advance!
