I posted about a very similiar situation that happened to my colleagues 
daughter about 6 months ago.  I'm sure it is in the archives.   It took 
several weeks to establish Lactogenesis ll.  She did go on to have a full 
supply.

I feel like the mothers body needs to heal itself and come out of it's own 
emergency state before it can produce any real volume of milk.

Subject: Sedation and breastfeeding


I have a request for information from an IBCLC colleague.  Her daughter 
delivered a baby girl October 21 and had a respiratory arrest and hemorrhage 
of about 1500-2000ml during the delivery, presumably from HELLP Syndrome and 
DIC.  She is still in Intensive Care and on a ventilator so is being sedated 
around the clock with Fentanyl and Versed.  She is also getting Lasix for 
edema and pulmonary fluid.  Her mom and sisters have been pumping her 
breasts regularly and are still getting colostrum with no increase in volume 
yet.  Baby is fortunate enough to be receiving banked human donor milk.
Her question is this:  Is the IV sedation having an effect on her prolactin 
and oxytocin levels.  Could damage to the pituitary gland be the reason 
there is no increase in volume yet?  I called the Infant Risk Center and 
they said that pumping should maintain her prolactin levels but she may need 
skin to skin to initiate the oxytocin response.  She is anxious and restless 
on the ventilator so that may have an effect too.
Any information you Wise Ones could share would be greatly appreciated, 
especially from Dr. Sarah if she is reading.
