I remember a dear LLL friend whose daughter the same age as mine about 3-4  
months old had "bad breath" for quite some time before an X-ray to rule out 
 sinus infection was done. The X-ray revealed that she had a small screw in 
 one of her nostrils. The tissue had grown around it, and was causing a low 
 grade infection. Big sister had put it in there unbeknownst to anyone.  It 
is unusual for a nursing baby to have bad breath, and this was occurring  
without any symptoms of a URI or a sinus infection. Luckily it was  able to 
be fairly easily removed during out-patient surgery. But of  course little 
one had to have general anesthesia for the surgery.
