In response to Julia:

I don't think I have ever looked for a "flanged lip". Primarily because if a mother tries to look for flanging of the bottom lip, she usually ends up dislodging the baby's lower lip from a deep latch by pushing her areolar tissue out of the baby's mouth in an effort to see it. If the jaw is fully opened, the neck in sufficient extension for an open airway for breathing while swallowing, and the lower lip is engaged deep in the areola tissue, you canNOT see the lower lip. I actually spend more time discussing how the mother feels when the baby is on rather than a visual assessment. Too many times I have heard mothers complain that someone told them the latch was beautiful and the person assisting them threw up their hands and basically gave the implied message that they had to suck it up and tolerate the pain. Meanwhile, that lower lip slid up. I do look for the nose sinking into the breast because usually (but not always) it means that the neck is flexed, the airway is constricted and the jaw is not as open.

Best, Susan Burger
