In our BF Center, we save our assessment forms and all f/u calls and

consults (f/u appts) for 1 yr. This is not part of the medical record.

We chart on moms and babies, but keep our own "assessment form" and f/u

log. I file it month by month by date of birth. Unfortunately, few moms

here bf long term. I have not had a problem keeping x 1 yr. I find it

very easy, when a mom calls, to find her "chart" when she gives me the

date of birth and her name. I can then see all the stuff that happened

in hospital and any f/u's we've had or phone calls. 

I file the current month in my desk drawer and the other months with the

most recent month in the front of the file cabinet, and going back the 1

yr.

At the end of the month when I file the month in my desk, do statistics

on it, and start a new month, I go in to the file cab. and throw the

month that is one year old in the shredder.
