I have quite a few little tongue thrusters. In many cases, the back rises up
in the process, and I have come to understand that it is often that part
that is kicking out the breast rather than the tongue tip. If you watch
really closely, there is often a posterior restriction that, as the baby
pushes the tongue forward, "catches" the mid section resulting in a lever
type action with the back rising up and the front sliding down. I'm probably
not explaining it very well but the more of these I watch as well as feel
with my finger, the better feel I get for what is happening.
Sometimes suck training exercises can work (see Cathy's book). Often times a
nipple shield helps, though you are likely to see it bob in and out of
baby's mouth; mom needs to hold baby in closely to minimize bobbing.
Frenotomy can help, depending on how complete the revision is. My quick 2
cents.
