Hi Malka -
Lots of thyroid references here: http://www.kellymom.com/health/thyroid/thyroid-refs.html
I believe that Breastfeeding: A Guide for the Medical Profession by Ruth Lawrence may have thyroid levels for a breastfeeding Mom listed - but I don't have a copy available to verify.
I don't have any references for "effect of no or minimal breast stimulation on day 2 on lactogenesis II and milk supply", but I've worked with many Moms who have been able to build a milk supply despite limited stimulation in the early days of lactation. Keep in mind that breastmilk production is under endocrine control through Lactogenesis II, so milk production is not a product of stimulation. Production control at the breast (autocrine) begins with Lactogenesis III.
Of course hypothyroidism has its own issues.
Nonetheless, no matter where a Mom is theoretically in the lactogenesis stages, health issues or whatever extent of lactation mismanagement has occurred, I don't make assumptions that Mom will not be able to obtain a full supply. We discuss possible interventions, create a plan that feels do-able to the Mom, and set a timeline to reassess. It's amazing to see some of the seemingly hopeless cases succeed.
