It is in situations like this that I am so glad I see mothers in my prenatal 
courses in addition to counselling them during breastfeeding.
In a recent case, a woman told me of her history of elective breast surgery 
in her teens and said that only much later did she realise that it could 
affect breastfeeding. So prenatally we discussed all the things she could do 
to establish her supply.
 It really helped that we had discussed it when she asked me to pay a home 
visit after the birth. We looked at the glass as half-full - and not 
half-empty - every drop that she could produce is a bonus.  With that 
attitude she put a lot of energy into getting her supply established but if 
there were days at the beginning when she had to supplement, she did not 
beat herself up about it.  She understood that it might take time but she 
was optimistic about it.
Within a month or so, she had sufficient to feed her baby without 
supplementing.
I shall be out of the loop for nearly a month, off to visit family in 
England and then to New York to have some quality time with our son and his 
family.
But I will get my e-mails most of the time so keep posting.
