I wonder if it's reflux. A baby can have reflux without spitting up at all. The regurgitation never gets that far, so it just really irritates the baby esophagus, which HURTS! I wouldn't want to eat either if it hurt every time!
A trial of some reflux meds, with the addition of some really good probiotics (like BioGaia) might be a good idea. If there is great improvement, then you know. I've seen this over and over again begin in babies about 4 weeks old or so. It's really worth a try.
