Hi All,
There is a difference between glandular hypoplasia and women with normal
glandular tissue who have poor glandular response to the hormone
environment.
Many women with glandular hypoplasia ( think bonsai tree) respond
appropriately to the hormones of pregnancy, delivery, prolactin, etc,
and will have engorgement, but the total amount of milk made is very
small due to having insufficient amount of glandular tissue.
Women with hormone issues such as PCOS have the potential to make enough
milk if the hormone situation is right. For example, if they lose a
bunch of weight before pregnancy such that their androgen levels are
lower going into pregnancy, they should respond better to hormones of
pregnancy and make more milk.
I is important to distinguish what the woman is dealing with.
Overstimulation of breasts that are responding well, as in glandular
hypoplasia, can lead to engorgement and mastitis.
