We have a note on our infant birth admission form that the nurse 
identifies all the factors at the birth:  infant dried, placed on 
abdomen skin to skin, apgar score, resuscitation, wt. etc.  As far as 
supplementation it is identified on the infants feeding record... any 
food given other then breast milk.  We chart skin to skin when assessing
