Posting with permission for colleague and her daughter. Mom was an attempted VBAC with resulting C-Sec. Approximate 1500cc blood loss at end of surgery not prior to birth. Mom went to ICU received 8 units of blood. Currently slightly anemic 9 hgb. Baby with as normal pp recovery as expected with a mom in ICU (baby not in NICU) no bottles given baby to breast every 3 hrs. during moms stay in ICU. Mom had resulting edema which is slowly resolving and now is in the lower part of her lower extremities. Baby is currently 10 days old & not vigourous at the breast. He is tongue tied which they are pursuing getting clipped. Peds doesn't want to do so they are calling an ENT today. Her first baby had a stretchy tongue tie that didn't need clipping. Baby has been being supplemented at the breast with an SNS at most feeds. Mom now doing switch nursing. Moms supply is slowly creeping up. Using a hospital grade EBP and pumping about 25 cc after feeds. Lots of skin to skin. Baby currently taking about 1oz at the breast using football hold with pre and post weights done by grandma (IBCLC). Mom is taking Reglan. Baby is gaining about 1 oz a day. They are hoping it is a tincture of time as moms body recovers from this insult. Any additional ideas or thoughts would be appreciated.
